Dr.
Mark Forrest, D.M.D., a native of Queens, New York received
his Dental Degree and his Specialty
training in Periodontics from New York
University, College of Dentistry.
Presently, he is a Clinical Associate
Professor at Nova Southeastern University,
College of Dental Medicine in the
Department of Periodontology.
Currently he is the Immediate Past President of the Florida
Association of Periodontists, and serves on its Executive
Board.

| Sofia was born in South Florida. She attended the Broward Community College Dental Assisting Program and graduated as a Dental Assistant. She also attended the American Intercontinental University and obtained an Associates of Arts degree in Business with honors. In 2004, Sofia was a volunteer for a non-for-profit organization in New York City. She enjoys spending time with her husband and family. Her hobbies are reading, swimming and taking her Welsh corgi for walks. Sofia is the proud mother of a new baby boy |

Diane was born in Freeport, NY and raised in New Jersey. She attended a school for Medical and Dental Assistants in New Jersey and graduated in 1975. Since then, Diane has been employed in the dental field in the areas of oral surgery, general dentistry, and orthodontics. After relocating to Florida in 1978,
Diane continued her career in the dental field. She is the proud mother of two grown children who are both enrolled at Florida International University. Her daughter is an education major and her son is a business administration major. In her free time, Diane enjoys decorating, party planning, sewing and reading. |

Shanika was born in a small town in
central Florida. She attended Broward
Community College and graduated as a
Certified Dental Assistant. Her hobbies
are singing, dancing and song writing. Shanika is also currently attending nursing school.

Liuva was born and raised in Cuba and moved to Florida in 1997. She has a Bachelors degree in Graphic Design from her native country. She desired and received a higher education degree in the United States. As a hygienist, Liuva loves her profession and likes to provide high quality care. In her spare time, she does community service for those in need. Liuva's hobbies are spending time with her family (especially with her three nephews), traveling, and going to the beach. |

Annette is thrilled to be part of the “Forrest Team”. She loves being a hygienist and takes pride in taking good care of her patients. Annette has worked for Dr. Forrest in the past as a certified dental assistant, and also holds a degree in marketing. She is currently studying for her Bachelor's degree in Health Science at Nova Southeastern University.
Arriving in Florida from Denmark at the age of fifteen, Annette knows how to appreciate the warm climate and the American culture. When not working, she treasures spending time with her three children.

Pilar graduated in 1997 from dental school in
Colombia, and has been working in the
dental field since then. Pilar has worked
with Dr. Forrest since 2002. Despite
her youthful appearance, Pilar was born
1975. She is married and a mother of
two beautiful girls.

our staff. Angela was born in New Orleans
and later relocated with her family to
South Florida. She is a certified Dental
Assistant. Angela enjoys spending time

Florida State University where he studied
Biochemistry. He received his dental
education at Nova Southeastern University,
College of Dental Medicine. Dr. Forrest

Surgery at the University of Florida.
Dr. Forrest is currently practicing Periodontics and Implant Surgery in Jacksonville, Florida.
