Memorial Hospital West Medical Office Building
The office of Dr. Mark Forrest is located in Pembroke Pines at 601 N. Flamingo Road, suite 318. We are in the Medical Office Building attached to Memorial Hospital West, just north of Pines Blvd. on the east side of Flamingo Road.

A parking garage is located in front of the medical office building. Use the FREE valet parking, or we recommend parking on the third floor and using the pedestrian bridge to get to our suite.

From I-95 or the Florida Turnpike, exit Hollywood Blvd., continue west and make a right turn on Flamingo Road. The Hospital will be on your right side.

From I-75 exit Pines Blvd. East. Continue and make a left on Flamingo Road. The Hospital will then be on your right side
